You need to:

1.     Fill in the parent agreement form with 3 site and the internship profile form and submit this to Dr. Faye.

You will also need to submit electronic copies of the following documents.

4.     ·  Passport photo

5.     ·  Emirates ID

6.     ·  Family book

DATE OF INTERNSHIP

JUNE 21- August 15 (holidays and work hours are as per your organization)
